Event description

One injured, one died while cleaning waste recovery tank

Investigation abstract

Employee #2 was cleaning a waste recovery tank and left to get some fresh air. L ater, he re-entered the tank. Employee #1 noticed that Employee #2 was feeling n auseated and went in to help him. Employee #1 fell into the tank. Both employees were trapped in the tank, ingesting the waste and inhaling the fumes, until the y were rescued by firemen and hospitalized. Employee #1 died several days later.
