FALL FROM SCAFFOLD

Investigation abstract

ON JULY 25, 1984, AT APPROXIMATELY 8:02 AM, EMPLOYEE #1 WAS WORKING ON A MANUALL Y PROPELLED MOBILE SCAFFOLD LOCATED AT THE TRINITY UNITED METHODIST CHURCH. HE W AS CLIMBING DOWN THE BUILT-IN LADDER WHEN HE FELL AN ESTIMATED 8-15 FEET TO THE CONCRETE FLOOR, FRACTURING RIBS ON HIS LEFT SIDE, PUNCTURING A LUNG AND CUTTING HIS LIVER. EMPLOYEE #2, A MASON, OBSERVED EMPLOYEE #1 AFTER THE FALL AND BECAME ILL. HE FAINTED AND FELL TO THE CONCRETE FLOOR, CUTTING THE BACK OF HIS HEAD. ST ITCHES WERE REQUIRED TO CLOSE THE CUT.
