Employee drowns after falling into creek
Investigation abstract
On March 8, 1989, Employee #1 and a coworker were sweeping up leaves in a parkin sked if anyone had seen him. Employee #1 had apparently fallen into the creek an d drowned. g lot. The coworker was sweeping and raking the leaves into a pile while Employe e #1 put the leaves in a container. The trash dumpsters were full, so Employee # 1 took the container of leaves to empty at the creek. The concrete-lined creek c ontained 4 to 6 ft of water and had a bank pitch of approximately 60 degrees. A 4 ft high chain link fence ran along its banks. When approximately 5 to 10 minut es had passed, the coworker looked up to locate Employee #1 and the container.
