Event description

Employee killed when thrown from basket of mobile platform

Investigation abstract

Employee #1 was unloading a JLG powered mobile platform from a trailer. The fron t wheels of the platform slipped, causing the rear wheels to hit the ground with such impact that the telescoping boom sprung up, throwing Employee #1 into the air. He landed on the guardrail of the basket. His weight pushed down the basket , which struck against the bed of the trailer. The boom then sprang up again, an d Employee #1 was thrown from the basket a second time. He again fell on the gua rdrail of the basket, and then tumbled approximately 8 feet onto the pavement. E mployee #1 was taken to the hospital, where he died of head and chest injuries.
