FALL FROM ELEVATED LOCATION

Investigation abstract

WHILE THERE WERE NO ACTUAL EYE WITNESSES TO THE ACCIDENT, IT IS APPARENT THAT EM PLOYEE #1 WAS EITHER WORKING ON THE ELEVATED SURFACE OF TANKS 4 AND 5 OR WALKING ON THE CATWALK. FROM ONE OF THESE PLACES, EMPLOYEE #1 FELL TO HIS DEATH. THERE WERE ONLY TWO OTHER EMPLOYEES AT THE PLANT AT THE TIME OF THE ACCIDENT.
