Event description

Employee killed when crushed by falling piece of steel

Investigation abstract

Employee #1 was using a carbon rod to air arc (cut) a large 1,200 to 1,300 lb pi ece of 5 ft 3 1/2 in. long by 15 1/4 in. wide by 5 1/4 in. high steel that was w elded onto the side of a 15-ton steel ladle. It was to be removed because the co mpany had a contract to recondition this ladle. The height was 5 ft at the low e nd and 5 ft 6 in. on the side to be rebuilt. A lifting hook had been welded onto the piece by another welder, as requested by his supervisor. Employee #1 did no t hook up the piece being cut to the lifting lug, and continued to cut the piece off the ladle. The piece fell, crushing him against the concrete floor. Employe e #1 was killed.
