FALL FROM SCAFFOLD
Investigation abstract
EMPLOYEE #1 WAS UNLOADING JAM BLOCK FROM A LIFT TRUCK WHILE STANDING ON A SCAFFO LD 17 FEET ABOVE GROUND LEVEL. THE SCAFFOLD WAS ERECTED ON ROOF TRUSSES. THE PLA NKING USED ON THE PLATFORM WAS NOT SCAFFOLD GRADE PLANK. EMPLOYEE #1 DROPPED A J AM BLOCK AND THE SHOCK LOAD CAUSED A STRINGER PLANK. TO BREAK, DROPPING THE SCAF FOLD AND EMPLOYEE TO GROUND.
