EMPLOYEE RIDING CRANE HOOK BALL FELL
Investigation abstract
ON OCTOBER 15, 1984, EMPLOYEE #1 HAD JUST FINISHED TYING TOGETHER REBARS IN A 40 FEET DEEP "CAN". HE AND ANOTHER EMPLOYEE CLIMBED ON THE BALL OF THE CRANE AND P ROCEEDED TO RIDE THE BALL TO THE TOP OF THE "CAN", FORTY FEET ABOVE THE GROUND. HE LOST HIS GRIP ON THE HOISTING CABLE AND FELL 40 FEET TO HIS DEATH.
