Employee killed in fall from scaffold

Investigation abstract

Employee #1 was painting roof trusses from a 12 ft 3 3/4 in. high by 10 ft long by 29 in. wide mobile scaffold manufactured by Up Right Scaffold, Inc., of Berke ley, CA. Employee #1 was killed when he fell to the concrete floor and fractured his skull.
