Employee killed in fall from wall
Investigation abstract
Employee #1 and coworkers were working atop a 9 ft 4 in. high by 8 in. wide by 1 5 ft long wall. Using a 2 in. concrete pump hose and a vibrator (110 ac), they w ere pouring concrete into cores of 8 in. by 8 in. by 16 in. plugs, three rows on each end of the wall. A coworker tried to pass the concrete pump hose to Employ ee #1, but knocked him off balance. Employee #1 fell to the ground, fracturing h is skull. He was killed.
