Employee crushed and killed in trench cave-in
Investigation abstract
Employee #1 entered an 8 ft deep by 42 in. wide trench to block the end of a pip e. A 2 ft thick by 20 ft long section of the left wall broke away, crushing and killing him. Rock had been blasted from the adjacent 16 ft section of the unshor ed trench.
