EMPLOYEE CRUSHED IN MULLERATOR MACHINE
Investigation abstract
EMPLOYEE #1 FINISHED CLEANING ONE MULLERATOR AND BEGAN CLEANING A SECOND ONE. EM PLOYEE #2, A MAINTENANCE WORKER, BEGAN WORKING ON THE FIRST MACHINE. EMPLOYEE #2 NEEDED TO MOVE SOME INSIDE ROTATING PARTS OF THE FIRST MACHINE, SO HE TURNED ON THE ELECTRICAL SWITCH, THEN WENT TO THE CONTROL PANEL TO ACTIVATE THE MACHINE. EMPLOYEE #2 PUSHED THE WRONG START BUTTON, ACTIVATING THE MACHINE EMPLOYEE #1 WA S WORKING ON BY MISTAKE. EMPLOYEE #1 WAS CRUSHED IN THE MACHINE'S MECHANISM. EMP LOYEE #1 HAD NOT LOCKED OUT THE SECOND MACHINE.
