Employee killed in fall when head strikes against palletizer
Investigation abstract
Between 10:00 p.m. and 10:15 p.m. on September 5, 1987, Employee #1 was clearing a jam in the elevator pit of a Columbia palletizer machine. He fell, struck his head against the machine, and was killed. The fall may have resulted from a hea rt attack or the inadvertent movement of machine parts.
