EMPLOYEE STRUCK AND KILLED BY FALLING STEEL COIL
Investigation abstract
Employee #1 was unloading a semi-trailer loaded with steel band coils which were sitting upright on wood cribbing. Employee #1 cut the steel banding holding the coils together, when two coils fell. One coil fell onto its flat side and trapp ed him inside its eye. The other coil fell over and struck him on the head and k illed him.
