FALL FROM SCAFFOLD
Investigation abstract
EMPLOYEE FELL 9FT (APPROX) FROM THE SECOND BUCK LEVEL OF A TUBULAR WEL- DED SCAF FOLD HE WAS ENGAGED IN ERECTING. HE WAS WALKING ON (2) TWO 2X10 INCH PLANKS. THE PLANKS HAD BEEN PREVIOUSLY INSTALLED ON (3) THREE OUTRIGGER BRACKETS--(1) ONE O F THE PLANKS WAS DISPLACED CAUSING THE EMPLOYEE TO FALL TO A CONCRETE FLOOR. THE SCAFFOLD PLANKS WERE NOT SECURED AND THE FALL RESULTED IN MULTIPLE FRACTURES TO BOTH OF THE EMPLOYEES ARMS.
