CUT BY SAW BLADE

Investigation abstract

ON ABOUT JUNE 2, 1986, EMPLOYEE #1 WAS INSTRUCTED TO CLEAN PIECES OF SCRAP WOOD OGRAM. FROM UNDER A SAW WHICH WAS USED TO CUT STOCK FOR ROOF TRUSSES. THE SCRAP TAKE-AW AY MECHANISM ON THE SAW WAS INOPERATIVE. THE EMPLOYEE WAS INSTRUCTED TO ACCOMPLI SH THE CLEANING BETWEEN CUTS, AND TO BE CAREFUL AROUND THE FOUR SAW BLADES. ON J UNE 10, 1986, EMPLOYEE #1 WAS REMOVING THE LAST SHOVELFUL OF SCRAP WOOD FROM UND ER THE SAW WHEN THE OPERATOR STARTED THE SAW. THE SAW BLADE CAUGHT THE SHOVEL HA NDLE AND DRAGGED EMPLOYEE #1'S RIGHT HAND ACROSS THE SAW BLADE. EMPLOYEE #1 RECE IVED A SEVERE LACERATION AT THE KNUCKLE JOINT AND THE PALM OF HIS RIGHT HAND.
