EMPLOYEE #1 WENT UNDERNEATH THE GARNETT MACHINE TO MAKE REPAIRS ON A CHAIN AND S PROCKET. HE WAS PULLED INTO THE GARNETT MACHINE AND WAS CRUSHED BY ROTATING CYLI NDERS. HE DIED IMMEDIATELY.
