Investigation abstract

A STACKER CRANE OPERATOR WAS FATALLY INJURED WHEN THE CARRIAGE HOIST CHAIN FAILE D AND THE CARRIAGE (INCLUDING THE OPERATOR'S CAB) FELL APPROXIMATELY 30 FEET TO THE FLOOR. EMPLOYEE #1, THE CRANE OPERATOR, SUFFERED MULTIPLE INJURIES.
