Employee killed when pinned between glass packs
Investigation abstract
Employee #1 was retrieving scrap wood from the top of stored cases of glass, eac h of which weighed 4,000 lb. Four unsecured cases tipped over, pinning him again st other cases of glass. The cases pressed against his chest and he died of asph yxia.
