Event description

Employee killed in fall from roof

Investigation abstract

Employee #1, a roofer, was utilizing stacks of roofing material to access anothe r roof at a higher elevation. When he stepped on an unstable part of the stored material, he lost his balance and fell approximately 10 ft 8 in. onto a concrete shipping dock. Employee #1 suffered multiple injuries to his head and chest, an d was killed. Causal factors of this accident include improper storage of materi als at the edge of the roof and the employee's decision to use this material as a short cut instead of using the access ladder provided.
