Investigation abstract

AN EMPLOYEE WAS REMOVING A TRANSFORMER COVER. THE COVER BLEW OFF THE TRANSFORMER , STRIKING THE EMPLOYEE IN THE HEAD. THE EMPLOYEE DIED OF HIS INJURIES.
