Investigation abstract

EMPLOYEE #1 STEPPED THROUGH A DOORWAY THAT HAD OPENED ONTO A ROOF AT ONE TIME. T HE ROOF HAD BEEN REMOVED, THE DOOR BARRICADEDAND A WARNING SIGN POSTED. EMPLOYEE #1 WAS KILLED BY THE FALL.
