EMPLOYEE STRUCK BY STAKE ATTACHED TO HEAVY EQUIPMENT
Investigation abstract
EMPLOYEE #1 WAS SHOVELING HOT MIX AT A HIGHWAY CONSTRUCTION SITE WHEN THE ACCIDE NT OCCURRED. THE LAYDOWN MACHINE WAS IMPROPERLY ATTACHED TO A 500-FOOT ROPE AND A STAKE DRIVEN INTO THE PAVEMENT. EMPLOYEE #1 WAS WORKING NEAR THE ROPE WHEN THE STAKE CAME OUT OF THE GROUND, HITTING HIM IN THE BACK OF THE NECK. EMPLOYEE #1 SUFFERED SEVERED ARTERIES AND SEPARATED BRAIN TISSUE AND DIED AS A RESULT OF THE SE INJURIES. THE PROCESS OF USING A GRADE LINE ATTACHED TO THE EQUIPMENT IS NOT PROPER OPERATION OF THE EQUIPMENT.
