CONSTRUCTION EMPLOYEES STRUCK BY VEHICLE ON HIGHWAY

Investigation abstract

EMPLOYEES #1 AND #2 WERE INSTRUCTED TO REMOVE TEMPORARY TRAFFIC DETOUR MATERIALS TH AT ABOUT 50 MILES PER HOUR FAILED TO OBSERVE THE REMAINING WARNING CONES (LOC ATED ABOUT 25 FEET SOUTH OF THE ARROWBOARD), RAN THROUGH THE CONES AND RAMMED TH E FLASHING ARROWBOARD. EMPLOYEE #1 WAS KILLED WHEN HE WAS CRUSHED BETWEEN THE AR ROWBOARD AND THE PICKUP TRUCK. EMPLOYEE #2 WAS KNOCKED UNDER THE BED OF THE TRUC K AND RECEIVED INJURIES FOR WHICH HE DID NOT NEED HOSPITALIZATION. (FLASHING ARROWBOARD UNIT AND CONES). THE ARROWBOARD UNIT HAD BEEN PLACED IN TH E EAST NORTHBOUND LANE OF TRAFFIC RATHER THAN ON THE SHOULDER OF THE ROAD AS THE Y SHOULD HAVE BEEN. EMPLOYEE #2 DROVE THE COMPANY-OWNED PICKUP TRUCK SOUTH AS EM PLOYEE #1 PICKED UP THE CONES AND PLACED THEM IN THE BED OF THE TRUCK. WHEN THEY REACHED THE FLASHING ARROWBOARD UNIT, EMPLOYEE #2 STOPPED THE TRUCK, TURNED, AN D BACKED UP TO THE UNIT IN ORDER TO HOOK IT TO THE TRUCK. EMPLOYEE #2 STOOD ON T HE WEST SIDE OF THE TRUCK WHILE EMPLOYEE #1 WALKED AROUND TO THE EAST. WHILE BOT H EMPLOYEES WERE BETWEEN THE TRUCK AND THE FLASHER UNIT A VEHICLE TRAVELLING NOR
