Employee killed when struck by sewer pipe during unloading
Investigation abstract
Employee #1, a driller, drove his truck loaded with sewer pipe to a construction site. He and a construction crew worker removed the bands and chains from the t hree tiers of pipes. A forklift operator removed four pipes from the right side of truck while Employee #1 was standing at the front of his truck. The forklift operator was removing the pipe from the top rear tier on the left side of truck when he saw one pipe from the front of the truck rolling off the right side. Emp loyee #1, who had walked around to the right side of the truck, was struck by th e pipe and killed.
