Investigation abstract

EMPLOYEE #1 WAS STANDING ON 35 FOOT 9 INCH HIGH PALLET OF MATERIALS. HE APPARENT LY STEPPED BACKWARDS OFF THE PALLET AND FELL TO THE CONCRETE FLOOR. HE WAS KILLE D WHEN HIS HEAD STRUCK THE CONCRETE FLOOR.
