FELL TO BOTTOM OF CHEMICAL REACTOR

Investigation abstract

EMPLOYEE #1 WAS ATTEMPTING TO REMOVE A METAL BEAM FROM THE LOWER PART OF THE INS IDE OF A CHEMICAL REACTOR. NO FALL PROTECTION OR LADDER WAS USED, AND FOOTING WA S VERY POOR. THE EMPLOYEE FELL 18 FEET TO THE BOTTOM OF THE REACTOR. THE VESSEL HAD BEEN PUMPED, AND SAMPLES TAKEN, PRIOR TO ENTRY. THE SECOND EMPLOYEE WAS ADMI TTED TO THE HOSPITAL FOR EXHAUSTION.
