Employee killed when struck by metal coupling
Investigation abstract
Employee #1 was handling a paint hose that was pressurized with low air pressure . He was holding the air hose at the end. When pressure was applied, the line be gan to whip and the metal coupling on the hose's end struck Employee #1 on the h ead, fracturing his skull and killing him.
