STRUCK BY THE BOTTOM PLATE OF A DRILLING BUCKET

Investigation abstract

EMPLOYEE #1 WAS STRUCK IN THE HEAD BY THE BOTTOM PLATE OF A DRILLING BUCKET. THE HINGE BROKE, ALLOWING THE 925 POUND PLATE TO SWING FREE AND STRIKE THE EMPLOYEE , KNOCKING HIM AGAINST THE CRANE TRACK. HE WAS HOSPITALIZED AND DIED A FEW DAYS LATER.
