AT APPROXIMATELY 10 AM ON JANUARY 22, 1986, EMPLOYEE #1 COLLAPSED AND DIED OF A MASSIVE HEART ATTACK. HE HAD FAILED TO TAKE HIS HIGH BLOOD PRESSURE MEDICATION F OR SEVERAL DAYS. NO VIOLATIONS WERE NOTED IN THE TUNNEL WHERE THE HEART ATTACK T OOK PLACE.
