EMPLOYEE KILLED WHEN HE WAS RUN OVER BY A FRONT END LOADER

Investigation abstract

ON DECEMBER 6, 1985, EMPLOYEE #1, WHO WAS TEMPORARILY OUT OF HIS USUAL WORK AREA , WAS STRUCK AND RUN OVER BY A FRONT END LOADER. HIS HEAD WAS CRUSHED. THE FRONT END LOADER WAS EQUIPPED WITH AN AUDIBLE ALARM WHICH ONLY FUNCTIONED WHEN IT WAS BACKING UP. THE MEDICAL EXAMINERS REPORT INDICATED THAT THE EMPLOYEE MADE AN ER ROR IN JUDGEMENT.
