Employee hospitalized after fall into excavation

Investigation abstract

Employee #1 was walking along the top beam of a bracing system for an excavation and slipped and fell approximately 22 ft. He caught a cross brace and hung to i t for a moment before falling to the top of a concrete pipe lying in the excavat ion. Employee #1 slid from the pipe and fell onto some 2 by 4s that were bracing forms at the bottom of the trench. Employee #1 was not using any fall protectio n and was hospitalized..
