EMPLOYEE CRUSHED BETWEEN TAIL GATE AND TRUCK BODY
Investigation abstract
AT APPROXIMATELY 2 PM ON DECEMBER 8, 1986, EMPLOYEE #1 WAS ATTEMPTING TO ACTIVAT E A "STUCK" ELECTRIC/HYDRAULIC TAIL GATE LIFT, LOCATED AT THE REAR OF AN ENCLOSE D STAKE BODY TRUCK. THE EMPLOYEE WAS FOUND WITH HIS HEAD CRUSHED BETWEEN THE ASC ENDING TAILGATE AND THE TRUCK BODY. THERE WERE NO EYEWITNESSES.
