Investigation abstract
EMPLOYEE #1 WAS TAKING A GRADE MEASUREMENT IN A 15 FEET DEEP TRENCH, LOCATED IN A LANDFILL, THAT WAS NOT SHORED OR SLOPED. HE SUFFERED FATAL INJURIES WHEN ONE O F THE SIDE WALLS COLLAPSED ON HIM. ANOTHER EMPLOYEE, WORKING FOR A DIFFERENT CON TRACTOR, WAS ALSO KILLED IN THIS ACCIDENT.
