EMPLOYEE #1 WAS WORKING IN A 15 FEET DEEP TRENCH IN A LANDFILL THAT WAS NOT SLOP ED, SHORED OR OTHERWISE PROTECTED FROM CAVE-INS. ONE OF THE BANKS COLLAPSED ON E MPLOYEE #1, KILLING HIM. AN EMPLOYEE FOR ANOTHER CONTRACTOR WAS ALSO KILLED IN T HIS ACCIDENT.
