Employee killed in trench cave-in

Investigation abstract

Employee #1 was in a 13 to 15 ft deep trench with vertical walls, searching for the tap-in location on a main sewer line where plumbers would attach a 4 inch li ne. He was killed when the south wall collapsed, burying him.
