Investigation abstract

EMPLOYEE #1 WAS CHANGING BOTTLES ON AN OXY-ACETYLENE WELDING CART. A HYSTER FORK LIFT, BEING USED TO REMOVE DEBRIS, BACKED INTO HIM. HE WAS PINNED AGAINST A WALL AND CRUSHED TO DEATH.
