Event description
One employee killed, one injured, in trench cave-in
Investigation abstract
Employees #1 and #2 were in a 21 ft deep trench installing 12 ft sections of 30 in. sewer pipe. While a section of sewer pipe was being lowered into the trench box, they stepped out into an unprotected area of the trench. The wall caved in, killing Employee #1 and completely burying Employee #2. Employee #2 was rescued and was hospitalized with a collapsed lung, bruises, and contusions.
