EMPLOYEE KILLED WHEN STEEL PANEL FELL ON HIM
Investigation abstract
EMPLOYEE #1 WAS SANDBLASTING A FREE STANDING STEEL PANEL. THE PANEL WAS 10 FEET LONG, 8 1/2 FEET WIDE AND 16 INCHES THICK. IT WAS COMPOSED OF TWO 8 INCH THICK P ANELS JOINED BY RIGID STEEL STRAPS EXTENDIND BEYOND THE PANEL THICKNESS. THE PAN EL WAS NOT SUPPORTED TO PREVENT IT FROM FALLING OVER. IT FELL ONTO EMPLOYEE #1 W HILE HE WAS SANDBLASTING. THE PROJECTING METAL STRAP PUNCTURED EMPLOYEE #1'S BAC K, KILLING HIM.
