Event description

Employee killed when head crushed by falling pipe

Investigation abstract

Employee #1 was unloading 20 ft lengths of a 24 in. pipe from a truck. When the "boomer" was released, a pipe or pipes rolled off the truck and crushed his head . He was killed.
