Employee dies after falling into elevator shaft
Investigation abstract
Employee #1 apparently opened the hoistway door of a freight elevator system whi le the elevator car was at another landing. He fell about 15 feet and apparently struck his head against the concrete basement floor. He died during surgery at a nearby hospital.
