Investigation abstract

EMPLOYEE #1 WAS INSTALLING A SIGN, WORKING FROM A PLATFORM SUPPORTED BY THE FORK S OF A CLARK FORK LIFT TRUCK. THE WORK PLATFORM TIPPED OFF THE FORKS, CAUSING TH E EMPLOYEE TO FALL 13 FEET TO THE FLOOR. HE WAS KILLED.
