Investigation abstract

EMPLOYEE #1 WAS CRUSHED TO DEATH WHEN A 1,100 POUND PAPER BALE FELL ON HIM. THE PAPER BALE HAD BEEN LOADED ONTO A TRAILER. EMPLOYEE #1 MAY HAVE PULLED THE BALE LOOSE HIMSELF.
