Event description
Employee killed in fall when scaffold overturns in wind
Investigation abstract
At approximately 3:30 p.m. on July 5, 1986, Employee #1 was attaching fuses on a Statue of Liberty display that was affixed to an approximately 39 ft high by 10 ft long by 5 ft wide scaffold. A gust of wind apparently blew the scaffold over . Employee #1 fell and was killed. The scaffold consisted of six sections of Saf -way open-end frame scaffolding with one plank being used as the work platform. It had no safe access. Electric fence wire was being used as a guy, and no type of fall protection was provided.
