Investigation abstract

EMPLOYEE #1 LOST CONTROL OF A TAMPO ROLLER-COMPACTOR AS HE CAME DOWN A STEEP INC LINE. HE HIT A WATER HYDRANT AND INLET, FLIPPING OVER TWICE. HE WAS THROWN FROM THE ROLLER, THEN STRUCK AND CRUSHED BY IT.
