EMPLOYEE #1 WAS UNDER A TRAILER, GREASING THE LEFT REAR AXLE. THE TRUCK DRIVER G OT INTO THE TRUCK, STARTED THE ENGINE, RELEASED THE BRAKES, AND PROCEEDED FORWAR D. EMPLOYEE #1 WAS RUN OVER AND KILLED. THE DRIVER WAS NOT AWARE THAT EMPLOYEE # 1 WAS UNDER THE TRUCK.
