Employee killed when loader rolls over on slope
Investigation abstract
At approximately 1:45 p.m. on August 10, 1987, Employee #1 was operating a JCB b the vehicle was equipped with one. ackhoe endloader, model 1400B, at a shopping center construction site. The backh oe was being used to dig mud from a retention pond and to spread it on the groun d. The backhoe was ascending a hill that also sloped down to the right when its drive wheels began to spin. Employee #1 lowered the outriggers and raised the ma chine clear of the ground. He then extended the backhoe boom and pushed the buck et into the ground to push the machine forward. When he raised the outriggers, t he loader rolled over and Employee #1's head struck against the rollover protect ion structure. He was killed. Employee #1 was not wearing a seat belt, although
