CRUSHED BY FALLING PIPE SECTIONS

Investigation abstract

ON MARCH 24, 1987, AT 10 AM, EMPLOYEE #1, WHILE ATTEMPTING TO REMOVE A LEG OF A TWO-LEG WIRE ROPE SLING, CROSSED UNDER A 5/8 INCH ROLLED STEEL PIPE. THE PIPE WA S TACK WELDED, USING EIGHT SECTIONS, INTO A 50 FOOT LONG PIPE. THE DIAMETER OF T HE PIPE WAS 54 INCHES. AS EMPLOYEE #1 PASSED UNDER THE PIPE, TACK WELDS ON 2 SEC TIONS FAILED AND THE PIPE SECTIONS FELL ONTO EMPLOYEE #1.
