Event description

Employee killed when struck by falling rack

Investigation abstract

At approximately 1:00 p.m. on August 13, 1988, Employee #1 was found on the floo r with severe body and head injuries, from which he died. There were no witnesse s to the accident. It is assumed that he was using a Towmotor to push a cantilev er steel rack that was approximately 8 ft wide by 15 1/2 ft long and weighed app roximately 2,500 lb. Near the east exit door, the rack became askew and, because there was not enough clearance to get the Towmotor to the other side of the rac k to straighten it, one end of the rack was raised by a crane. The Towmotor move d to the other end and while the rack was being lowered to the floor, the rack b ecame disengaged from the hook of the steel chain sling and struck Employee #1.
