TWO EMPLOYEES KILED WHEN VALVE ON STEAM LINE EXPLODED
Investigation abstract
EMPLOYEES #1 AND #2 WERE SHUTTING VALVES IN A STEAM LINE SO THAT WORK COULD BE D ONE ON THE LINE. ONE OF THE VALVES EXPLODED SOON AFTER BEING CLOSED, KILLING BOT H EMPLOYEES.
